Each team ‘Covid Marshall’ must complete the form below by 5pm day before play. It is an HPA requirement that all teams must have a nominated Covid Marshall who must collate team information and submit the completed form.
Attendees at an HPA Affiliated Club or HPA Organised Polo Event. Attendees are limited according to the Government guidelines. Currently, neither supporters nor spectators may attend.
Each attendee must:
- Contact Information. Provide to the club either via app, email or in writing their contact information and self-declaration that they are symptom free having checked their own temperature the day before and the morning of attendance. If it is 37.8 or above, they should not go to polo.
- Use the NHS QR Code Check on arrival.
